Overview
Coming home after a hospital stay, or managing a long-term condition, involves a great many small arrangements. Our coordinators keep them in order: appointments, reminders, records, and the right people informed.
Where skilled clinical care is needed, we refer to appropriately licensed providers and coordinate around their visits. We do not present ourselves as a clinical provider, and we never replace medical advice.
